Before your Appointment
- Must bring consent form from Middleton Outreach Ministry to first visit
- 24-hour cancellation policy
- A "no-show" may result in exclusion from this program
- Lab fees may apply
- $100 charge may be applied to patients being seen on a non "share a smile" day
